Word of the Week: HCP

A Health Care Proxy is a document which lets you appoint someone you trust to take on responsibility for your health related decisions should you lose the ability to speak for yourself.  Everyone over the age of 18 should appoint a health care proxy and it is important to discuss who this is and any major issues or concerns with your primary health care provider.  This is a vital part of ensuring that your health wishes are carried out if you become unable to verbalize them.
